Case Managers assess each client and arrange for jointly agreed on services for the client(s) and caregiver(s). 

They coordinate service delivery among various agencies, individuals and others. 

They assist clients and caregivers in obtaining available benefits, services, entitlements, necessary home modifications, or other services required to fulfill unmet needs. This allows the clients to remain at home in the safest, most comfortable environment possible

Case Managers seek to maximize appropriate volunteer and other unpaid services before arranging for formal (paid) services. The case manager and client, or the client’s legal representative, work together to develop a supportive service plan that will establish both the frequency and duration of needed services.
